What TMS costs in Washington and how insurers approve it

Nearly every major Washington plan covers TMS for depression. The hold-up is almost never coverage — it is documentation.

Washington's largest insurers cover TMS for major depressive disorder, but all require documented failure of prior treatment. Premera Blue Cross, Regence BlueShield, Kaiser Permanente, UnitedHealthcare, Aetna and Cigna typically require trials of two or more antidepressants — and often a course of psychotherapy — before approving TMS, with prior authorization handled through each plan's behavioral health team. Apple Health (Washington Medicaid) covers TMS through its managed care plans, with coverage criteria that vary by plan. Medicare covers TMS statewide under the Noridian local coverage determination. Premera Blue Cross

One of Washington's largest insurers. Covers TMS for treatment-resistant depression with prior authorization, usually requiring documented trials of two or more antidepressants.

Regence BlueShield

Covers TMS for treatment-resistant depression with prior authorization and medication-trial documentation.

Kaiser Permanente Washington

Covers TMS for major depressive disorder that has not responded to medication and therapy; referrals go through your Kaiser care team.

UnitedHealthcare

Covers TMS under the behavioral health benefit; authorization is typically handled by Optum rather than the medical plan.

Aetna

Covers standard rTMS and, on many plans, Deep TMS. Requires medication-trial documentation and a current depression rating scale score.

Cigna

Covers TMS for treatment-resistant depression. Prior authorization and re-authorization partway through the course are required.

Apple Health (Washington Medicaid)

Coverage runs through managed care plans such as Molina, Amerigroup, Coordinated Care, Community Health Plan of Washington and UnitedHealthcare Community Plan — confirm with both the plan and the clinic.

Medicare (Noridian Jurisdiction F)

Covers TMS for major depressive disorder under the Noridian local coverage determination, including many Washington Advantage plans with their own authorization step.

TRICARE West

Covers TMS for eligible service members and families with referral and authorization through TriWest.

Getting approved faster

Bring a list of every antidepressant you have tried, with doses and how long you took them.
Ask your prescriber to note the reason each medication was stopped — side effects count.
Have a recent PHQ-9 or similar score on file; most insurers want a current measure.
Let the clinic submit the authorization — their staff do this daily and know the templates.
If you are denied, appeal. Denials for incomplete documentation are routinely overturned.
